DHS Survey about Home and Community Based Services
Please take the opportunity to take a survey and tell the Minnesota Department of Human Services (DHS) about community services for individuals with brain injuries and how well they work. If you have a brain injury or are a caregiver for someone who has a brain injury, please take the time to take the survey and have your voice heard.

To take the survey:

DHS wants to know:
  • What kind of help you receive?
  • Does this help meet your needs?
  • Do you need help that you cannot get?
  • What makes it difficult to get help you need?

The survey is confidential. All of your answers will be private. No one will know who took the survey or what they said. All who complete the survey will be invited to enter a drawing for one of five $100 gift cards to Target or Walmart.

The deadline has been extended, so please complete the survey by May 15, 2015.
