The Case Management program provides case management services for individuals who are on a Traumatic Brain Injury (TBI) or Community Alternative for Disabled Individuals (CADI) Waiver and have a brain injury diagnosis. Waivers, such as the TBI and CADI Waivers, are funding sources from the government. Waivers provide the opportunity for individuals to live in the community rather than in nursing homes or institutionalized settings.
Case managers at the Minnesota Brain Injury Alliance are contracted by various counties in Minnesota to monitor and implement a system of support for individuals. Working closely with each participant, the Minnesota Brain Injury Alliance case managers use the Waiver funds to coordinate a tailored service plan, identify and connect with service providers, and regularly review the support plan in place to ensure that each individual receives the highest level of customized service possible. The Waiver funds can be used to pay for services and supports such as independent living skills, homemaking, personal care services, supported employment, transportation, behavioral services, or day program fees. Each person's needs and vision of a better quality of life is unique, so, accordingly, each plan of support organized by Case Management is different.
